On September 29, 1989 the Northbound Amtrak Crescent struck a fire truck at a crossing with crossbucks only near Catlett, Virginia as it was responding to a call. 11 of the 16 cars derailed. There were three fatalities, all with the fire dept. 150 injuries to passengers.

My dad gave me a couple pictures he took of the cleanup.
