As I stated in another thread a few days ago, my dad was a trainorder collector, as well as being an op/telegrapher and a train order copier himself. Here's a couple or three blanks that he got somewhere, these weren't used orders, but they are rare forms from unusual railroads.

Here's an order blank from the U.I.C. I don't know much about the UIC, I believe that it was a juice line in northern Utah and southern Idaho. If a railroads locomotives didn't boil water, or use diesels in the 1940's or 1950's, I don't usually pay them much mind.
