Another safety violation, this time endangering the inspectors who are out there because of previous safety violations.

I don't like jumping on outrage bandwagons, but wow, this is unbelievable.

Yet again, evidence of what happens when a transporation agency is run
by incompetent bureaucrats whose only merit lies in skill at picking
the public's pocket.

The other Metro outrage article notes that the controllers in the operations center are now certified, for the first time since 2012.  And they still have trouble communicating with track workers, etc in many tunnels.

That was an issue in the Yellow Line fire / smoke condition two years ago.  The DC fire fighters didn't trust the ROC had shut down the power and wouldn't go in.

The operator failed to read back the slow order, and the control center allowed the train to proceed?

That sounds like it's consistent with lack of a safety culture

That is usually just a part of other corporate culture problems. In my experience, if the corporate culture problem is not a problem limited to the employees (generally the case), the safety gig ends as soon as the result of the investigation into the root of the problem is presented.

I have been on jobs in which we were just another in a string of consultants who came up with the wrong answer...and the search for someone to give the right answer went on. I have had employees ask what makes us think we can change anything when we're among the many and nothing has changed.
