In Australia - It's usually two. Typically they're both fully qualified, and they share the driving duties. Some lines and self-contained systems have "driver-only" working, and my current employer is determined to move to remote operation with no traincrew on the locomotive at all.

Russia is two-man crews as well (both freight and passenger).
