But how the switch ended up the way it was is an obvious focal point for investigation.

Because a wayside switch indicator is hard to install (the line has them at the Honda switch)... PTC is a federal mandate that tries to prevent human error...trouble is, humans always will error...even with PTC. We got some big pentagon targets in the yard and asked if they would put them on a few north facing switches as we are directional on the main. No way. Costs too much. Instead, the local leaves a switch open and a loaded coal train smacks a standing cut of loaded covered hoppers near track speed. Both crew injured, one bad.
My math says the signs were cheaper and easier to see at distance. Might have lessened the damage. Local ran through their own switch again. Same crew. We have zones now with rail integrity announcements. And they balked about wiring in the switches even after above wreck happened. Then, the train went in the yard at speed instead of going down the main. The lead unit went halfway through a loaded covered hopper, which was the first in a standing cut of cars. All of the jacknifing and rollovers are the result of the fairly high speed (40 MPH?) collision on curves... G |
