Train 89 was routed down a previously closed track, killing two veteran MW employees.

Redundant  and automatic protection for guys on the ground, including shunting devices is recommended

They can't blame lack of PTC on this one.

The link goes to the executive summary.   Read the entire report  RAR-17-02 a PDF download found on the right. 

Way too lax.  Night foreman wanted to go home, day foreman late..

Night foreman....I'll take down the fouls but you have to call right away and request them.   Then he walks away, w/o following to insure it is done. 
Train dispatcher talking to his wife on comapny phone describing   "something doesn't quite look right".

Guys on the ground have the right to expect their supervisor has an interest in their safety.
