Early signs are pointing to "driver error" and not a failure of the automatic train control system, the cause of the deadly Ft. Totten crash in 2009.

I doubt it's a failure of the automatic train control system considering that was disabled right after the Ft. Totten crash and has yet to reactivated...

ATC has several sub-systems. It's ATO (Automatic Train Operation) that was discontinued after Ft. Totten. It has come and gone on a trial basis since then. ATC is the umbrella term for speed enforcement, train separation, etc., that is still very much in operation. 

ATC allows operators to proceed at restricted speed after a stop indication, under certain conditions. But, apparently, this operator failed to follow protocol and did not receive clearance from the control center to do so.
