Tram either split a switch or hit a swtich that was set against the tram.

The tram seemed to travel a good distance off the tracks

That tram looked to be traveling at a good rate of speed.
Maybe it was going way too fast to take that turn?

I'll not try to determine what caused this but that was happening at what seems to be a very high speed for regular operation of a street opersted tram resulted in an awful crash.

It seems like the operator assumed a straight route, but switch points aligned for the turn.
