I read the initial reporting (in Japanese) hours after it happened, and as in the English report you linked, miscommunication was brought up.

I've watched how engineer and track workers interact during my scout-by-ride sessions by being in front of the train. Most busy main lines have track occupancy lights that flash when trains are approaching - I've used that to anticipate incoming trains coming around the corner for my videos. Track workers use that to stop work and stand away from the tracks, and as a matter of procedure, they are aware even before the flagger waves the flag to communicate visually with the engineer who honks (as mentioned in the article).

Don't know if this section of Tobu Railway's Nikko Line had those track occupancy flashing lights. Shin-Kanuma is not considered a busy train station. Speculation is that the herbicide sprayers might have lacked experience in dealing with trackside safety. The Spacia X did stop at Shin-Kanuma before departing and striking the workers.
