For those of us that are not familiar with coupling mechanisms, what is the problem with the pic?

Well, let's see... What's wrong with this picture?

a) Couplers are painted - makes it hard to find cracks
b) Both are top lift pin styles, which are more difficult, and thus more dangerous, to operate
c) Red one has no lift pin - missing
d) Red coupler is attached directly to frame - no draft gear to cushion shocks
e) Red coupler has no swivel to allow for misalignment or sharp curves
f) The OP title indicates the humor of the shot - there is no alignment control (that part of the draft gear in the frame which keeps couplers in alignmemt when slack runs in to prevent buckling of the train.

Anyone see anything else?

Not a problem, you just shove it in front of the lead unit as crossing protection.
