You can see the elevation difference and the guide bars in that second shot, not the best situation
for low flying objects.

That appears to be a "flange bearing diamond", which it seems I have read are not nice to steam engines. Based on your original post "something" went wrong; any clues for the rest of us?

From what I understand a sanding pipe snagged on the diamond and was shoved back up into the firebox causing some minor damage that was quickly repaired.
