While not quite East Coast, 765 is running trips in Indiana Oct 7-8th and 14th-15th.

There are two narrow gauge steam options that come to mind - the East Broad Top in PA is running Fri, Sat, Sun with three trips each day, and the Wiscassett, Waterville and Farmington in Maine is operating one day each weekend Sept 30 - Oct 21.  Plus the Mt. Washington Cog Railway in New Hampshire runs one or two steam trains daily.

Cass is 12 miles to the Bald Knob summit, and then there is also the rebuilt line to Durbin.

In the East, but not near any coast is the Everett Railroad in Pennsylvania.  Its Autumn Adventure trip between Hollidaysburg and Martinsburg is being offered October 6, 9, 13 & 20.  This is a 3-hour/27-mile round trip.  More information can be found at this website: https://everettrailroad.com/autumn-adventure.

Based on my ride a similar steam special operated in April 2018, I'd recommend this excursion to anyone.  With fall colours at the peak in this part of Pennsylvania during the second and third weeks of October, this should be a very scenic ride.
