The DC Metro is returning to full Automatic Train Operation (ATO) 5 years after a collision caused by a fault in the signal system killed 9 people.

Anyone interested in PTC should read the docket items on the NTSB website.
It is a real eye opener. Similar accidents will occur on the Class 1's as PTC equipment ages.

DC's management also authorized non-endorsed software fixes, installed non compliant track segments, and otherwise operated work arounds for the manufacturer's recommendations, too. Stuff ages and breaks, but it needs to be replaced with authorized equipment.
