#2 has poo poo'ed last sleeper and is being setout

Cajon says they are setting out the last sleeper on a siding between Tuson and Maricopa due brakes not releasing and a water leak, train had gone in emergency, had to drag at 20 mph to spur for setout. Passengers moved to lounge car. Decision to be made in Tuson as to what to do. Cajon offered to call LA Rail to see they can pick them up in Tuson.

when an unfortunate situation like this happens, does Amtrak have a follow up procedure?

Was this condition (any condition similar to this) written up in the recent past? Go through the last ten etc trips, and see if there was a complaint? Was it resolved, and signed off as fixed? Or did somebody say the problem wasn't found, and the car released for service?

What would happen, if anything, if the condition was reported, but not fixed? Somebody said they replaced something, somebody signed off on the work, but nothing was really done, and the old part is still on the car.

Are there any penalties, or does somebody lose vacation time, suffer a loss of pay, etc?

Absolutely.

And sometimes things happen which can be prevented from happening in the future. Just saying "S%&* happens" doesn't cut it in most places.

Amtrak has had great results from its reliability centered maintenance process for the Acela equipment It's unfortunate the company chose not to apply it to the diesel shops, car shops, etc.

This was more to the idea of a freak incident which this sounds like... No air brakes and no water sounds like a piece of debris may have struck the undercarriage of the car, resulting in a loss of both Brake Pipe and Main Reservoir pressure resulting in the car needing to be set out. I've seen a few trains where cars in the consist have to be dropped because of the remains of a "Slow Elk" (Bovinus Stupidus) rolling underneath at 79.

The sleeper had air problem when it was in the LA shop. They could not get the brakes to release or the air cut out on the trucks. A long bus ride from Tucson to San Antonio 16 hours for the spare sleeper. All of this happen at Maricopa and the car was set out just east of Maricopa.

Car was flood with water so no water and brakes problem. This car need a major overhaul.

16 hours to the spare sleeper for train 22. Train sold out and 30 people in the Chicago sleeper.

They could not get the air cut out on the trucks so they set the car out.
