Most people using a lens of that length don't have camera around their neck, but rather over the shoulder. What I can see this as useful would not be for rail fanning, but rather for when you are out in crowds, this would tend to make the camera much more secure to someone lifting it off your shoulder.

It allows total freedom of motion, and eliminates the concern of a camera slipping off your shoulder. I've worn it hiking into the Grand Canyon, and have also worn it horse back riding, where it was terrific since there was a lot of up and down motion.
