Apparently this is to represent the whistle-stop campaign trips she made with her husband. The US Mint website has details.

Evidently there were six entries for the Bess Truman reverse, and three of them were train-related.

Odd note shown within the text for the coins at the Mint: " A minimum of four one-half ounce, $10 24-karat gold coins will be minted and released annually in the order the spouses served in the position."
